Job Summary
The Licensed Vocational Nurse (LVN) plays a crucial role in delivering direct patient care within a home health environment, executing care plans developed by a registered nurse. This position focuses on ensuring high-quality care and effective communication among patients and healthcare professionals.
Responsibilities
- Implement care plans established by registered nurses.
- Provide direct patient care in a home health setting.
- Maintain accurate documentation of patient information and care activities.
- Educate patients and their families about care protocols and health management.
- Coordinate with a multidisciplinary healthcare team to deliver comprehensive services.
Qualifications
- Graduation from an accredited vocational nursing program.
- A minimum of three years of nursing experience is required.
- Must hold a valid state nursing license.
- Possession of a current and valid driver’s license.
- Experience with a reliable, insured vehicle for transportation.
- Current CPR certification is mandatory.
