The LPN is responsible for monitoring resident health, maintaining care plans, and overseeing medication administration procedures. They also facilitate communication between physicians, staff, and families to ensure high-quality resident care. Candidates must hold a current LPN certification in the state of Colorado. A minimum of one year of experience in home health, assisted living, or long-term care is preferred.
