Job description
**Job Title: Social Worker**
The Social Worker is responsible for delivering case management, crisis intervention services, and community resource coordination for patients and their families within a home health environment. This role includes participation in interdisciplinary care meetings and the upkeep of clinical records to promote patient health objectives and facilitate discharge planning. Candidates must possess a Master’s degree in Social Work. A minimum of 1-2 years of experience in a home health or hospital context is essential, along with a valid driver’s license and CPR certification.
