Job description
**Licensed Practical Nurse (LPN)**
The Licensed Practical Nurse (LPN) delivers hands-on patient care and clinical assistance, encompassing tasks such as monitoring vital signs, administering medications, and conducting diagnostic tests in a concierge primary care environment. Additionally, the LPN is responsible for managing chronic care programs, ensuring the accuracy of electronic health records, and facilitating detailed care plans for a designated patient group. Applicants should be graduates of an accredited Practical Nursing program and possess a valid LPN license for the state in which they are practicing. A minimum of two years of nursing experience in a clinical setting, along with a current certification in Basic Life Support (BLS) from the American Heart Association, is required.
