Job Summary
The Medical Social Worker plays a vital role in delivering psychosocial assistance, conducting evaluations, and coordinating care for patients and their families in home settings. This position involves working closely with an interdisciplinary team to tackle emotional, social, and environmental challenges while facilitating smooth transitions in care.
Responsibilities
- Provide psychosocial support and assessments to patients and families in their homes.
- Coordinate care and collaborate with interdisciplinary teams to address various psychosocial factors.
- Ensure effective transitions of care for patients and families.
- Identify and address emotional, social, and environmental challenges affecting patient care.
Qualifications
- Master’s or Doctorate degree in Social Work.
- Active LMSW or LCSW license.
- At least 1 year of experience in healthcare social work.
- Valid driver’s license and current CPR certification.
