The Medical Social Worker will conduct psychosocial assessments and evaluate the needs of patients and their families in a home health setting. They will provide counseling, connect patients with community resources, and coordinate care with an interdisciplinary team. Candidates must hold a Master’s Degree in Social Work (MSW) and possess a full, active, and unrestricted social work license. Additionally, applicants are required to have a valid driver’s license, a reliable personal vehicle, and current auto insurance.
