**Mission Case Manager**
The Mission Case Manager collaborates with individuals seeking services to create and execute tailored service plans while linking them to appropriate support services and community resources. Key responsibilities include maintaining precise documentation, tracking client progress, and representing the organization’s mission positively. Applicants should possess at least two years of higher education or similar experience, in addition to a minimum of three years in case management or direct support roles. Competency in Microsoft Office applications and a valid driver’s license paired with dependable transportation are essential.
