Serve as a point of contact for patients, families, caregivers, physicians, and community members, coordinating appointments, access to services, and care across the healthcare continuum. Collaborate with multidisciplinary teams to educate patients, support safe care transitions, identify at-risk patients and community needs, and track outcomes to improve care. Must hold an active Ohio Licensed Practical Nurse or Registered Nurse license, speak fluent Pennsylvania Dutch, and demonstrate cultural competence and a commitment to serving the community. BLS certification is required, and ACLS certification is required or must be obtained; the candidate should also be organized, flexible, communicative, able to prioritize, and willing to travel as needed.
