Job Summary
Matrix Medical Network is on the lookout for a Nurse Practitioner or Physician Assistant (PRN) to conduct thorough in-home health evaluations for members of a health plan in Yorktown, Virginia. This part-time position requires you to perform comprehensive health assessments.
Responsibilities
- Conduct detailed patient histories and physical examinations.
- Perform risk assessments to identify care gaps.
- Utilize findings to develop patient care strategies.
- Deliver high-quality, patient-centered care in home settings.
Qualifications
- Active Nurse Practitioner or Physician Assistant license.
