Job Summary
Matrix Medical Network is in search of a Nurse Practitioner or Physician Assistant located in Lynchburg, Virginia. This position involves performing detailed in-home health evaluations for members of a health plan, focusing on understanding their medical background and current health status.
Responsibilities
- Conduct thorough reviews of patients’ medical histories.
- Administer physical examinations to assess the health of patients.
- Reconcile patients’ medication lists to ensure accuracy.
- Identify and address gaps in patient care to enhance health outcomes.
Qualifications
- Relevant certification and training as a Nurse Practitioner or Physician Assistant.
- Experience in conducting health assessments and managing patient care.
- Strong interpersonal skills for effective interaction with patients and health plan members.
