Job Summary
The Nurse Practitioner or Physician Assistant in Family Medicine – Float Pool is responsible for providing comprehensive patient care under the guidance of the Medical Director or an approved physician. This role collaborates closely with primary care coordinators (PCCs), nursing staff, and discharge coordinators within the clinical setting in Lafayette, Louisiana.
Responsibilities
- Deliver high-quality medical care to patients in a float pool capacity.
- Work closely with the Medical Director and nursing staff to ensure effective patient management.
- Collaborate with other healthcare professionals to support patient discharge and transition processes.
- Engage in patient education and promote health maintenance practices.
- Maintain accurate and detailed patient records in compliance with healthcare regulations.
Qualifications
- Must possess a Master’s degree in Nursing or a relevant field for Nurse Practitioners.
- For Physician Assistants, a Bachelor’s degree in Physician Assistant medicine or Health Sciences is required.
- Current licensure as an APRN or Physician Assistant in the state of Louisiana.
- Demonstrated ability to work effectively in a team-oriented environment.
- Strong communication skills to interact with patients and healthcare team members.
