Job Summary
The Palo Alto Foundation Medical Group is in search of a dedicated and certified Nurse Practitioner or Physician Assistant to join their Family Medicine team in Dublin, California. This full-time position emphasizes providing comprehensive healthcare services to a diverse patient population.
Responsibilities
- Provide direct patient care as part of the Family Medicine team.
- Conduct thorough assessments, diagnose conditions, and implement treatment plans.
- Work collaboratively with physicians and other healthcare professionals to ensure quality patient care.
- Educate patients and their families on health management and disease prevention.
- Document patient encounters and maintain accurate medical records.
- Participate in continuing education and professional development activities.
Qualifications
- Active certification as a Nurse Practitioner or Physician Assistant.
- Experience in a family medicine or primary care setting is preferred.
