Job Summary
We are seeking a dedicated Nurse Practitioner or Physician Assistant to join our team as a Full-Time State Traveler based in Richmond, Virginia. This position requires effective onsite engagement to ensure quality care delivery in various settings, including patients’ homes and Skilled Nursing Facilities.
Responsibilities
- Provide comprehensive health assessments and develop care plans for patients.
- Facilitate health education and wellness initiatives for patients and their families.
- Coordinate with physicians for referrals and case management services.
- Travel to various locations to meet patients as required.
Qualifications
- Current Nurse Practitioner or Physician Assistant licensing in the state of Virginia.
- Experience in healthcare, preferably in a home health or SNF setting.
- Strong interpersonal and communication skills to effectively collaborate with healthcare teams.
- Ability to travel as needed within the state.
