Job Summary
We are seeking a dedicated Nurse Practitioner or Physician Assistant to join our team on a PRN basis in Machias, Maine. This role focuses on providing comprehensive healthcare services, including treatment and health education, to patients in various settings, including their homes and Skilled Nursing Facilities. This position offers an excellent opportunity to impact patient care while working with a skilled team of healthcare professionals.
Responsibilities
- Deliver patient-centered care through assessments, diagnosis, and treatment.
- Provide health education and promote wellness to patients and their families.
- Facilitate physician referrals and case management as necessary.
- Work collaboratively with a multidisciplinary team to ensure optimal patient outcomes.
Qualifications
- Valid Nurse Practitioner or Physician Assistant license.
- Experience in primary care or outpatient settings is preferred.
- Strong communication and interpersonal skills to engage with patients and families.
- Ability to manage time effectively and work independently in various settings.
