Job Summary
The position of Nurse Practitioner or Physician Assistant (PRN) focuses on conducting in-home health assessments within the Queensbury, New York area. This role offers an exciting opportunity to impact patient care by providing essential evaluations and health education directly in patients’ residences or Skilled Nursing Facilities.
Responsibilities
- Perform comprehensive health assessments for patients in their homes or nursing facilities.
- Provide health education to patients and their families.
- Facilitate physician referrals and case management as necessary.
- Collaborate with healthcare teams to ensure optimal patient outcomes.
- Maintain accurate and timely documentation of patient care and assessments.
Qualifications
- Current certification as a Nurse Practitioner or Physician Assistant.
- State licensure to practice in New York.
- Strong assessment and diagnostic skills.
- Excellent communication and interpersonal abilities.
- Experience in home health or community-based settings is preferred.
