Job Summary
The position of Nurse Practitioner or Physician Assistant (PRN) focuses on delivering in-home health assessments in Monticello, New York. This role entails providing comprehensive healthcare services to patients, ensuring they receive appropriate treatment, health education, and referrals as needed. The ideal candidate will work on-site, participating in a dedicated team to enhance patient care and promote their well-being.
Responsibilities
- Conduct in-home health assessments for patients, gathering necessary medical histories and performing physical examinations.
- Provide treatment plans, health education, and referrals to physicians or other healthcare professionals as required.
- Engage in case management to ensure continuity of care for patients within their homes or Skilled Nursing Facilities.
- Collaborate with other healthcare team members to optimize patient outcomes and address specific health needs.
Qualifications
- Valid Nurse Practitioner or Physician Assistant licensure in New York State.
- Experience in primary care, home health, or long-term care is preferred.
- Strong communication and interpersonal skills to effectively interact with patients and their families.
- Ability to work autonomously and manage patient care independently.
