Job Summary
The Nurse Practitioner or Physician Assistant (PRN) will be engaged in conducting in-home health assessments, offering essential healthcare services directly to patients in their residences or in Skilled Nursing Facilities throughout Skowhegan, Maine. This position emphasizes the delivery of quality care and effective communication to support patient health and well-being.
Responsibilities
- Perform comprehensive health evaluations for patients in their homes or designated facilities.
- Provide treatment options and health education tailored to individual patient needs.
- Facilitate physician and case management referrals as necessary.
Qualifications
- Certification and active licensure as a Nurse Practitioner or Physician Assistant.
- Experience in delivering in-home health care or similar environments is preferred.
