Job Summary
We are seeking a Nurse Practitioner or Physician Assistant for a PRN role focused on conducting in-home health assessments in Calais, Maine. Responsibilities
- Perform thorough health assessments for patients in their homes or at skilled nursing facilities.

- Facilitate physician referrals and coordinate case management to ensure comprehensive care.
Qualifications
- Valid Nurse Practitioner or Physician Assistant certification and licensure.
- Experience in conducting patient assessments, preferably in home health or geriatrics.
