Job Summary
The Nurse Practitioner or Physician Assistant (PRN) will conduct in-home health assessments in Machias, Maine. This role involves providing essential healthcare services to patients in their residences or in Skilled Nursing Facilities. The professional will be integral in offering treatment options, health education, and facilitating physician referrals and case management.
Responsibilities
- Conduct in-home health assessments for patients.
- Provide treatment and health education tailored to individual patient needs.
- Facilitate physician and case management referrals as necessary.

Qualifications
