Job Summary
The position of Nurse Practitioner or Physician Assistant (PRN) for In-Home Health Assessments is designed for healthcare professionals who are dedicated to providing quality patient care in the Lancaster, Virginia area. The role involves delivering health evaluations and assistance either at patients’ homes or in Skilled Nursing Facilities, with a focus on enhancing their wellbeing through medical guidance and support.
Responsibilities
- Conduct comprehensive health assessments and evaluations for patients in their residences or Skilled Nursing Facilities.
- Deliver treatment plans and provide necessary health education tailored to individual patient needs.
- Establish essential referrals for physician consultation as well as for further case management.
- Ensure continuity of care through effective communication with patients and healthcare team members.
Qualifications
- Current licensure as a Nurse Practitioner or Physician Assistant in the state of Virginia.
- Demonstrated experience in performing health assessments and managing patient care.
- Strong communication skills for effective interaction with patients and healthcare teams.
- Ability to work independently and make clinical decisions within a home health setting.
