Job Summary
The position of Nurse Practitioner or Physician Assistant (PRN) is focused on conducting in-home health assessments in Broadway, Virginia. This role is integral to enhancing patient care through thorough evaluations and referrals within the healthcare system.
Responsibilities
- Conduct comprehensive in-home health assessments to evaluate patient needs.
- Provide health education to patients and their families.
- Facilitate physician referrals and case management when necessary.
- Deliver quality care within patients’ homes or Skilled Nursing Facilities.
Qualifications
- Current certification as a Nurse Practitioner or Physician Assistant.
- Experience in performing patient assessments and providing care in home settings.
- Strong communication skills to educate and collaborate with patients and caregivers.
- Ability to work independently and manage patient care effectively.
