Job description
The provider will assess, diagnose, and oversee the treatment of cardiovascular issues among patients in skilled nursing and long-term care settings. They will work alongside interdisciplinary teams to formulate treatment strategies and ensure comprehensive clinical documentation is maintained. Applicants must possess a valid, unrestricted Nurse Practitioner or Physician Assistant license along with national certification. Additionally, a minimum of six months of clinical experience and a current driver’s license for daily travel are necessary.
