Job Summary
The role involves performing detailed in-home health evaluations for seniors and individuals at risk, focusing on uncovering chronic health issues and addressing gaps in care. Providers will work closely with primary care doctors, caregivers, and other healthcare professionals to guarantee effective care coordination and maintain thorough documentation.
Responsibilities
- Conduct thorough in-home health assessments for older adults and at-risk individuals.
- Identify chronic conditions and manage care gaps effectively.
- Collaborate with primary care physicians and caregivers to ensure seamless care delivery.
- Maintain precise documentation related to patient care and assessments.
Qualifications
- Master’s degree in a relevant field.
- Current Nurse Practitioner licensure with prescriptive authority.
- Board certification in an appropriate specialty area.
- Current certifications in BLS, ACLS, or CPR.
