Job Summary
The role involves performing thorough in-home health evaluations for elderly and at-risk populations to detect chronic illnesses and address care disparities. This position emphasizes the importance of teamwork with primary care providers, family caregivers, and social work professionals to deliver cohesive healthcare and maintain accurate patient records.
Responsibilities
- Conduct detailed health assessments in the homes of older adults and other at-risk individuals.
- Identify chronic health issues and work on closing care gaps.
- Collaborate effectively with primary care doctors, caregivers, and social workers to ensure coordinated healthcare delivery.
- Maintain clear and precise documentation of patient care and assessments.
Qualifications
- Master’s degree in a relevant field.
- Active Nurse Practitioner license, including prescriptive authority.
- Board certification in an appropriate specialty.
- Current Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), or CPR certification.
