Job Summary
The role involves conducting thorough in-home health evaluations for senior citizens and at-risk individuals to detect chronic illnesses and enhance the quality of care provided. This position requires collaboration with primary care physicians, caregivers, and healthcare professionals to effectively manage patient health and ensure appropriate referrals are made.
Responsibilities
- Perform comprehensive health assessments for older adults and individuals at risk.
- Identify chronic health conditions affecting patients.
- Enhance the quality of care through systematic evaluations.
- Collaborate effectively with healthcare teams, including primary care doctors and caregivers.
- Manage patient health and provide necessary referrals to specialists.
Qualifications
- Master’s degree in a relevant field.
- Current Nurse Practitioner licensure with prescriptive authority.
- Board certification in a relevant specialty area.
- Current certifications in BLS, ACLS, or CPR.
