Job Summary
The role involves conducting thorough in-home health evaluations for elderly and high-risk individuals, focusing on their medical background, adherence to medications, and social challenges. The provider will work in tandem with primary care professionals and caregivers to facilitate patient education, offer referrals, and maintain precise clinical records.
Responsibilities
- Perform detailed in-home health assessments for geriatric and at-risk patients.
- Evaluate medical histories and medication compliance.
- Identify and address social risk factors affecting patients’ health.
- Collaborate with primary care physicians and caregivers.
- Provide patient education on health management and wellness.
- Issue referrals as necessary.
- Prepare and manage clear and accurate clinical documentation.
Qualifications
- Master’s degree in a relevant field.
- Current Nurse Practitioner license with the authority to prescribe medications.
- Board certification in an applicable specialty.
- Current certifications in BLS, ACLS, or CPR.
