Job Summary
This position involves the execution of thorough in-home health evaluations aimed at older adults and individuals at risk, focusing on identifying chronic health issues and enhancing overall health results. The role necessitates teamwork with primary care physicians, caregivers, and social workers to promote coordinated care and maintain accurate records.
Responsibilities
- Conduct extensive in-home health assessments for seniors and at-risk populations.
- Identify chronic health conditions and areas for health improvement.
- Collaborate with healthcare professionals, including primary care doctors and social workers, to ensure seamless care delivery.
- Maintain precise and comprehensive documentation of health assessments and care plans.
Qualifications
- Master’s degree in a relevant field.
- Active Nurse Practitioner licensure with the authority to prescribe medication.
- Board certification in an appropriate specialty.
- Current certifications in BLS, ACLS, and CPR.
