Job Summary
The role involves conducting detailed in-home health evaluations for elderly patients and individuals at risk, aiming to detect chronic health issues and enhance care results. This position requires collaboration with primary care doctors, caregivers, and other healthcare professionals to guarantee proper follow-up and record-keeping.
Responsibilities
- Perform thorough in-home health assessments for seniors and at-risk populations.
- Identify and manage chronic conditions to improve patient care outcomes.
- Work closely with primary care physicians and healthcare teams to coordinate patient follow-up.
- Maintain accurate documentation related to patient care and assessments.
Qualifications
- Master’s degree in a relevant field.
- Current Nurse Practitioner license with prescriptive authority.
- Board certification in a pertinent specialty.
- Current certifications in BLS, ACLS, or CPR.
