Job description
**Nurse Practitioner**
The Nurse Practitioner will deliver empathetic care and facilitate care coordination for patients in partnership with physicians and a multidisciplinary healthcare team. Key responsibilities include the diagnosis and management of both acute and chronic medical conditions, conducting comprehensive physical assessments, and maintaining precise documentation in the electronic medical record system. Applicants are required to possess a Master’s degree from an accredited program in Adult or Family Nurse Practitioner studies or Physician Assistant programs. A minimum of two years of clinical experience in geriatric, adult, or family practice environments is essential, and candidates must have active state licensure and board certification.
