The Registered Nurse will be responsible for assessing patient inquiries and performing home visits to deliver hospice and homecare assistance. This role involves working in conjunction with a multidisciplinary team to ensure precise documentation and empathetic care for both patients and their families. Applicants should have an active RN license in New Hampshire and access to dependable transportation. A Bachelor’s degree in nursing is desirable, and prior experience in home health care or hospice environments is preferred.
