The LPN will coordinate referrals for chronic care conditions and administer prescribed medications and treatments under physician supervision. They are also responsible for maintaining accurate patient medical records and communicating effectively with the healthcare team and patients. Candidates must hold a Vocational/Technical Diploma in Practical Nursing and a valid LPN license in Virginia. A minimum of 2 years of nursing experience in a hospital or clinic setting and current BLS certification are required.
