**Job Title: Outreach Specialist**
The Outreach Specialist is responsible for recognizing and supporting homeless or at-risk veterans by facilitating connections to vital resources and performing psychosocial evaluations. This role involves collaborating within a team to engage in community outreach efforts, keeping precise documentation, and liaising with local organizations to ensure veterans have access to housing and supportive services. A high school diploma or GED is required, with a preference for candidates holding a bachelor’s degree in social work or a similar discipline. A minimum of one year of experience in delivering psychiatric rehabilitation services to individuals who are disabled, low-income, or experiencing homelessness is essential.
