**Outreach Specialist**
The Outreach Specialist is responsible for implementing targeted initiatives aimed at engaging patients to address gaps in both chronic disease management and preventive healthcare. This role entails working closely with care teams and practice personnel to ensure service coordination, documentation audits, and enhancements in overall health outcomes. Applicants should possess a high school diploma or its equivalent and have a minimum of 2 years of experience either as a medical assistant or in a population health environment. Familiarity with EMR systems is essential, and candidates who have completed an accredited program for medical assistant, LPN, EMT, or ER technician are preferred.
