Job description
The practitioner will evaluate patients in Assisted Living Facilities (ALF) and Skilled Nursing Facilities (SNF) to deliver palliative care and pain management services. Duties encompass executing procedures, prescribing medication, and maintaining documentation of treatment plans in patient records. Applicants must hold a valid Nurse Practitioner or Physician Assistant license in Kansas and possess an active DEA license for this role.
