Job Summary
The Patient Care Coordinator is responsible for managing the journey of patients within the healthcare system, ensuring they receive optimal care and support from the point of entry through to the completion of their treatment. This role plays a pivotal part in enhancing patient satisfaction by facilitating effective communication between patients and healthcare providers while streamlining services offered.
Responsibilities
- Assist in assessing patient needs and coordinating appropriate care plans.
- Act as a liaison between patients, families, and clinical staff to facilitate communication.
- Schedule appointments, follow-ups, and any necessary treatments for patients.
- Manage patient records and ensure documentation is accurate and up-to-date.
- Provide support and guidance to patients regarding their treatment options and process.
- Ensure compliance with healthcare regulations and organizational policies.
Qualifications
- Bachelor’s degree in healthcare, social services, or a related field is preferred.
- Experience in a healthcare setting, with knowledge of patient care practices.
- Excellent communication and interpersonal skills.
- Strong organizational abilities and attention to detail.
- Proficiency with healthcare management software and systems.
