**Job Title: Patient Navigator**
The Patient Navigator plays a crucial role in delivering on-the-ground assistance to homeless individuals diagnosed with HIV/AIDS, helping them secure access to medical care, housing, and various social services. Key responsibilities include overseeing medication adherence, accompanying clients to their appointments, and meticulously recording all services provided in a case management system. To qualify for this position, candidates are required to have at least an Associate’s degree. It is highly recommended that applicants have prior experience working with homeless communities, individuals affected by AIDS, or those with substance use challenges.
