**Job Title: Patient Navigator**
The Patient Navigator plays a crucial role in linking community individuals to vital health and wellness resources while serving as an advocate for patients throughout their medical visits. This position involves offering informal counseling and keeping comprehensive records of patient interactions to facilitate seamless care coordination. Applicants should have a high school diploma along with two years of experience, an associate degree, a bachelor’s degree, or four years of experience in a related field. A background in customer service and community outreach is strongly preferred.
