Job Summary
The Patient Transition Liaison plays a crucial role in ensuring the smooth transition of patients among home health, palliative, and hospice care services. This position focuses on understanding patient requirements, fostering collaboration with clinical teams, and nurturing connections with referral partners to guarantee optimal care delivery.
Responsibilities
- Assess patient needs and circumstances to facilitate care transitions.
- Coordinate with clinical teams to ensure comprehensive care planning.
- Develop and maintain strong relationships with referral partners.
- Ensure effective communication channels are established for all parties involved in patient care.
Qualifications
- A minimum of two years of experience in healthcare or a related marketing field.
- A bachelor’s degree is preferred.
- Exemplary communication and interpersonal skills.
- Strong organizational abilities and the capability to work independently.
