Job Summary
The Patient Transition Liaison plays a crucial role in ensuring seamless patient care by assessing patient requirements and managing the transitions among home health, palliative, and hospice services. This position prioritizes the establishment of robust relationships with clinical teams and referral sources while ensuring that information is shared accurately and admission goals are achieved.
Responsibilities
- Evaluate patient needs to facilitate transitions between various care services.
- Maintain effective relationships with clinical teams and referral partners.
- Ensure the accurate flow of information related to patient care.
- Meet established admission quotas, contributing to the organizational goals.
Qualifications
- Minimum of two years’ experience in healthcare or marketing.
- Strong interpersonal and communication abilities.
- Bachelor’s degree is preferred.
- Capability to work independently.
- Familiarity with new technologies and their applications in the role.
