Job description
**Job Title: Physical Therapist**
Deliver personalized physical therapy to senior patients in diverse home and community environments by creating tailored care plans. Work alongside an interdisciplinary team and ensure accurate documentation of patient progress within the electronic medical record (EMR) system. To qualify, candidates must have a valid Physical Therapy license or be in the process of obtaining one, along with a degree from an accredited Physical Therapy program. Essential skills include proficiency in fundamental computer operations, experience with EMR systems, and a current CPR certification.
