Job Summary
The Physical Therapist will evaluate the conditions of patients and create tailored rehabilitation programs aimed at enhancing mobility and functionality. This role also includes working alongside a diverse healthcare team to guarantee excellent care and uphold precise clinical records.
Responsibilities
- Assess patient conditions for appropriate rehabilitation needs.
- Design individualized treatment plans to improve patients’ mobility and overall function.
- Administer specific therapies and treatment modalities as prescribed.
- Collaborate with an interdisciplinary team to ensure comprehensive patient care.
- Maintain accurate and timely clinical documentation of patient progress and treatment outcomes.
Qualifications
- Must possess an active and valid Texas Physical Therapist license.
- Good standing with relevant regulatory boards is required.
- Experience in a Skilled Nursing Facility or long-term care setting is advantageous.
- Proficient in the use of electronic documentation systems.
