Job Summary
The role involves providing in-home physical therapy services under the guidance of a Physical Therapist to assist patients in enhancing their mobility, strength, balance, and overall functional independence.
Responsibilities
- Conduct in-home therapy sessions to support patient rehabilitation.
- Educate patients and their caregivers about the therapy process and self-care techniques.
- Accurately document therapy sessions in the WellSky/Kinnser system.
- Regularly communicate patient progress and any concerns with the healthcare team.
Qualifications
- Completion of an accredited Physical Therapy Assistant program.
- Possession of a valid California Physical Therapy Assistant license.
- Current CPR/BLS certification.
- Access to reliable transportation and willingness to travel to patient residences.
- Preference given to candidates with experience in home healthcare and knowledge of Kinnser/WellSky software.
