Job Summary
The provider will offer essential primary care services to individuals residing in skilled nursing and long-term care facilities. This role encompasses conducting physical examinations, diagnosing various health conditions, and working alongside interdisciplinary teams to create personalized care plans. Additionally, the provider will play a key role in educating both patients and families regarding treatment objectives.
Responsibilities
- Deliver primary care services to residents in skilled nursing and long-term care settings.
- Perform comprehensive physical exams and diagnose health issues.
- Collaborate with interdisciplinary teams to formulate tailored care plans.
- Educate patients and their families on treatment goals and procedures.
Qualifications
- Possession of an active and unrestricted license as a Nurse Practitioner, Physician Assistant, or Physician in Ohio.
- A minimum of 6 months of experience working in a long-term care or skilled nursing facility environment.
