Job Summary
The role involves delivering essential primary care services to individuals residing in skilled nursing and long-term care establishments. This position requires conducting physical assessments and diagnosing various health issues. Additionally, the provider will work in partnership with interdisciplinary teams to formulate personalized care strategies while also providing education to patients and their families.
Responsibilities
- Provide primary care services to residents in skilled nursing and long-term care facilities.
- Conduct physical examinations and diagnose health conditions.
- Collaborate with interdisciplinary teams to create tailored care plans.
- Educate patients and their families about health management.
Qualifications
- Must possess an active and unrestricted license as a Nurse Practitioner, Physician Assistant, or Physician in the state of Ohio.
- A minimum of 6 months experience in a long-term care or skilled nursing environment is required.
