**Job Title:** Primary Care Provider for Geriatric Patients
Deliver proactive primary healthcare services aimed at adults aged 65 and above, emphasizing the management of chronic conditions. Collaborate effectively within a multidisciplinary care team that includes Nurse Practitioners, Physician Assistants, and social workers to provide quality, value-driven medical care. Candidates must hold Board Certification or Board Eligibility in either Internal Medicine or Family Medicine. An ability to work within a value-based care framework and excellent communication skills are essential for this role.
