Job description
Job Title: Primary Care Provider for Adults 65+
Deliver preventive primary care and manage chronic diseases for individuals aged 65 and above, focusing on a value-driven healthcare model. Work alongside an interdisciplinary care team that includes Nurse Practitioners, Physician Assistants, medical assistants, and social workers to ensure comprehensive patient care coordination. Applicants should possess Board Certification or be Board Eligible in Internal Medicine or Family Medicine. While experience in geriatrics and value-oriented care is advantageous, it is not mandatory.
