Job description
Title: Geriatric Care Specialist
Deliver preventative healthcare and manage chronic illnesses for adult patients aged 65 and above according to a value-driven care model. Work in conjunction with a multidisciplinary care team, including Nurse Practitioners, Physician Assistants, social work professionals, and pharmacy specialists, to enhance patient well-being. Applicants should possess Board Certification or be Board Eligible in Internal Medicine or Family Medicine. While prior experience in geriatrics and a background in value-based care is favorable, it is not a mandatory requirement.
