**Job Title: Case Manager**
The Case Manager is responsible for orchestrating interdisciplinary care for patients and conducting evaluations of admission documentation to guarantee optimal use of resources. They will also evaluate discharge requirements and foster connections with payers to support quality clinical and financial results. Applicants need to have a professional license valid in the state of employment and must hold an up-to-date BLS certification. A minimum of three to five years of experience in an inpatient environment, including acute care, IRF, or LTACH, is necessary.
