Job description
**Registered Nurse Care Manager**
The Registered Nurse Care Manager oversees discharge planning, facilitates transitions of care, and manages case management tasks to achieve the best possible outcomes for patients. This role involves working closely with interdisciplinary teams to assess medical records for care appropriateness and ensure adherence to regulatory requirements. Applicants are required to have an Associate Degree in Nursing along with 2-4 years of clinical nursing experience in settings such as hospitals, home health care, or nursing facilities. A current Registered Nurse license in the state of employment or a participating Compact state is mandatory.
