**Job Title: Care Transition Coordinator**
Facilitate effective transitions of care by acting as the main connection among various clinical teams to guarantee smooth recovery for patients. Develop tailored discharge processes and champion essential patient resources while assessing the precision of clinical documentation. A valid Registered Nurse license in Maryland or a compact state is required, along with a BLS certification from the American Heart Association. Applicants should possess a minimum of three years of experience in case management, and a bachelor’s degree in nursing is highly preferred.
